Thank you for your recent purchase of a Kidz Kit. In today's ever changing and dangerous society, the Kidz-IDz Child Photo ID & Medical Emergency Card and Kidz-IDz Electronic ID will help keep your child safe, and will assist you and others in caring for your child in the event of a missing child episode or medical emergency.

To get the ID Card and Electronic ID that you purchased, please click here to download and print the Enrollment Form. While completing the form, please print clearly, and provide estimates if necessary. Enclose the completed form in an envelope with your child's photo (optional) and the shipping & handing fee. Affix postage and mail to: Kidz-IDz Card Processing Center, PO Box 4110, Cherry Hill, NJ 08034.

Your Kidz-IDz child Photo ID & Medical Emergency Card, along with your original photo, and any other products you may order, will be mailed to you within the next four to six weeks. The welcome letter that accompanies your order will include instructions on how to access the Kidz-IDz Electronic ID.
