In today's ever changing and dangerous society, the Kidz-IDz Child Photo ID & Medical Emergency Card and the Kidz-IDz Electronic ID will help keep your child safe, and will assist you and others in caring for your child in the event of a missing child episode or a medical emergency.

Each ID Card is customized with the child's photo, important personal data, medical conditions, allergies, three emergency contact phone numbers, and an optional medical release section.

Furthermore, the Electronic ID is recommended as the fastest solution for transferring key information in the event of a missing child episode or a medical emergency. It enables you to save the child's photo and descriptive details in a useful electronic format, and includes other helpful files to help prevent and react to an emergency.

You may either complete the "On-line Enrollment Form" below, or you may select "Printable Enrollment Form" to enroll through the mail. Either way, there is a nominal shipping & handling fee of $3.95, but the $2.00 processing fee is waived for on-line orders. If enrolling through the mail, please follow these instructions:

• Complete each section. Please print clearly, and provide estimates if necessary.
• Select a current photo of the child and write his/her name on the back (the child's photo will be returned with your new ID Card). If you do not provide a photo, the ID Card will be produced without a picture.

Note that safety experts recommend that all families have three ID Cards for each child in the household. The first and second should be carried by mom and dad or another caregiver, so that each has a picture and descriptive information to share with the police and/or medical emergency personnel in the event of a missing child episode or a medical emergency. The third should be carried by the child (younger children may affix the ID Card to a breakaway safety lanyard/necklace, while older children may choose to carry it in the book bag or pocket) during family activities - visits to the park, playground, mall, vacations, etc.

Your Kidz-IDz Child Photo ID & Medical Emergency Card(s), and any other products you may have ordered, will be mailed to you within the next four to six weeks. The welcome letter that accompanies your order will include instructions on how to access the Kidz-IDz Electronic ID.
